Anaethestists registration documents
As a preferred-provider under the National Contract, we are required to request a number of documents from Anaesthetists to register for locum work in the UK. We aim to make registration as simple as possible and are always available via telephone or email for assistance.
In order to register you will need to read our Terms & Conditions and complete and return the registration documents. These can be downloaded in PDF format below (you will need Acrobat Reader to view these files).
We also need to post you a Criminal Records Bureau (CRB) Disclosure Application form – please telephone or email so this can be sent to you straight away or, if you prefer, enter your details on the registration enquiry page and we will contact you.
